Download and complete the appropriate form(s) below, then email them to info@summerleaguesports.com.

Scoresheets

Print one of these out for each game and ask a volunteer for your team to fill it out. Use tick marks to record aces for each player. At the conclusion of each set, record the score for each team in the lower section.

For Individuals

Player Registration Form

Required for every player who wants to participate in the 2026 season. One form per player — parents or guardians should complete and sign on the player's behalf. Players must have a completed form on file to be eligible on game day.

Coach Registration Form

Required for all volunteer coaches. Complete one form per coach and submit it along with your team registration.

For Teams

Team Registration Form

One form per team, completed by the head coach. Includes the full player roster and gets your team added to the schedule. Submit this form along with individual player and coach forms.
